SPECIMEN AFFIDAVIT FOR CHANGE OF NAME

(On non-judicial stamp paper of prescribed value)

  1. That my name recorded in school and college documents is __________________________, whereas I am generally known as ___________________________________ (new name).
  2. That through this affidavit, I have changed my name from _________________________ (old name) to _____________________________ (new name), and hereafter I shall be recognised as ______________________________ (new name) for all official and personal purposes.

(This affidavit must be signed and attested in the presence of a Judicial Magistrate, Executive Magistrate, Notary Public, or Consular Officer at an Indian Mission abroad.)

Note: For a change of name, the applicant must publish advertisements in two reputed newspapers—one local newspaper of the present residence area and one newspaper of the permanent address area—and submit the original copies of both advertisements while applying to the Institute.
