Why and When is it needed?
- Colleges take names according to enrolment details. After marriage, many women change surnames.
- Employers, governments, agencies, or banks might ask for documents under this new name.
- The sworn affidavit will be needed by the university to prove that both before and after the marriage the applicant is the same person.
Example:
- A woman joins a government job post-marriage and needs her service book to match her updated surname.
-
While applying for higher studies, a mismatch in certificates and Aadhaar name requires university verification.
How to Change Your Name in University Records:
- Affidavit Preparation: As per the above format, a notarised stamp paper.
- Gather Supporting Documents:
- Marriage Certificate.
- ID Document with updated name (Aadhaar, Passport, Voter ID).
- Old ID Document (with maiden name).
- Submit Application to Registrar: Attach an affidavit and documents with a written request.
- University Verification: The Registrar cross-checks details with past exam records.
- Certificate Issued: University issues a formal Name Change/Correction Certificate for official use.
Checklist of Documents
- Notarised affidavit.

- Aadhaar or ID proof with the new surname.
- Copy of degree/mark sheet with maiden name.

Changing the Name in School Records
The name change after marriage (after school):
- Schools usually do not alter old mark sheets or certificates
- You can apply to CBSE/State Board for the name change certificate or for the correction note only.
Required documents:
- Affidavit
- Marriage certificate
- Proof of identification showing new name
- Application to Principal/Board
Correction during school years (spelling mistake/wrong entry):
- Parents should make the application to the Principal to the Education Board.
- Board accepts the school admission register + affidavit + supporting ID.
- If corrected in time, new certificates/marksheets would be issued with the corrected name.
General name change (not marriage):
- Most of the requirements for a general name change by CBSE and state boards are Gazette notification and newspaper publication.
- Affidavit + Gazette copy + newspaper clippings will be submitted through school to the Board.
Key Difference
University (marriage name change): No newspaper needed; an affidavit + marriage proof is enough.
School (general name change): Gazette + newspaper publication compulsory (as per CBSE/ICSE/State Board by-laws).
