Why this affidavit is used
- This affidavit is required when a person's passport is lost, misplaced, or damaged, and they need a reissue of the passport. It serves as a sworn declaration explaining the circumstances of loss/damage and assures the Passport Authority of the applicant's responsibility.
 
When it is used
- If your passport is lost (whether in India or abroad).
 - If your passport is damaged (torn, water-damaged, or unreadable).
 - If you have lost passports earlier (multiple times), it is mandatory to declare details.
 - When applying for a duplicate/replacement passport with the Passport Seva Kendra/Consulate.
 
Purpose of this affidavit
- To officially declare the details of loss/damage (how, when, where).
 - To confirm whether you travelled on the lost passport and entry details.
 - To disclose if you availed TR concessions/foreign travel allowances.
 - To give details of residence abroad (if NRI).
 - To confirm, no objection by Passport Authority was recorded.
 - To declare if you were ever deported and expenses repaid.
 - To undertake responsibility for safe custody of the reissued passport.

(This affidavit is to be executed on non-judicial stamp paper of minimum value in the presence of First Class Judicial Magistrate or Executive Magistrate/Notary Public on the following points.)

- State how and when the passport was lost/damaged and when the FIR was lodged, at which Police Station and how many passports were lost/damaged earlier.

 - State whether you availed yourself of any TR concessions/FT allowances, and if so, details thereof?
 - State whether non-resident Indian and, if resident abroad, the details of the residence as follows:
