Affidavit for One and the Same Person

Last updated: Type: Affidavit Format Type: Affidavit Fill the Affidavit

**Sample PDF of the Affidavit is attached at the end.

Why this affidavit is used

To clarify and legally confirm that different versions of a person's name (spelling variations, expanded/shortened forms, initials, etc.) appearing in different documents all belong to the same individual.

Example:
In Aadhaar: Ravi Kumar Singh
In Passport: Ravi K. Singh
In PAN Card: Ravi Singh

This affidavit clears the confusion officially.

When it is used

  • While applying for a passport, visa, or immigration (to prove identity).
  • During banking/financial transactions, if a name mismatch is found in PAN, Aadhaar, or bank account.
  • In property transactions where the buyer/seller's name differs in the sale deed and ID proofs.
  • For government exams, service records, pension, or PF withdrawal when names don't match across documents.
  • In educational records where the student's name differs in school certificates vs. Aadhaar.

Purpose

  • To remove doubts about identity mismatch across official records.
  • To provide a legal declaration accepted by government offices, banks, courts, and embassies.
  • To avoid rejection/delay in applications due to name variations.

FAQs

Q1. Is this affidavit legally valid proof of identity?

Yes, it is accepted by government offices, courts, and foreign embassies, provided it is executed on stamp paper and notarised.

Q2. Can it be used instead of name correction in documents?

No. It only clarifies identity, but it does not permanently change/correct the name in official records. For correction, you need a Gazette Notification or relevant authority's process.

Q3. Who can swear this affidavit?

The individual concerned (whose name differs).

Q4. Is it needed for a passport if the names of parents differ across documents?

Yes, sometimes a parent's "One and the Same Person" affidavit is also required.

Q5. Is it enough for property registration if names differ?

Generally yes, but some Sub-Registrar offices may also ask for supporting ID proofs or Gazette publication.
