Why is this affidavit used?

  • To declare and confirm your residential address when no other valid proof is available or when authorities specifically ask for it.
  • It works as a self-declaration on oath before a notary or magistrate.

When is it used?

  • Government applications → PAN card, Passport, Voter ID, Aadhaar corrections, Ration Card, Driving License, etc.
